The Tier 2 weight management programmes are 12-24 week exercise and lifestyle programmes often provided by local councils, consisting of a variety of group sessions and occasionally 1:1 sessions as well. The first 12 weeks are the standard offer, with the second 12 weeks being dependent on a 5% weight loss. They are always free and have the criteria of 18+ and a BMI of over 30, or 25 where there are additional health risk factors present.

The Adult Tier 3 Weight Management Service, also referred to as the Specialist Weight Management Service (SWMS), is a multi-disciplinary, intensive, secondary-care based programme, designed to support adults with severe obesity and complex needs who require a more individualised approach than the Tier 2 service has previously been able to offer them.

The programme is typically 6-12 months and will potentially include input from a physician (either consultant or GP with a specialist interest), specialist nurse, specialist dietitian, psychologist, psychiatrist and  physiotherapist. You can view the CCG's commissioning statement, here.

Primary Care colleagues in the S&R and HaRD localities are being asked to pause referrals into the Tier 3 weight management service for North Yorkshire. The number of referrals into the service, provided by South Tees Hospitals NHS Foundation Trust, has exceeded expectations, and as a result there is no more capacity to see patients until a solution can be found. Any referrals now sent for the programme for S&R and HaRD patients will be returned by the Trust, and the referral option will no longer be visible through the eRS and RSS referral systems.

The Integrated Care Board appreciates this will come as a disappointment to many of you. We are actively trying to find a solution and will inform you if the position changes.

In cases of clinical exceptionality, Tier 4 access may be available via the individual funding request route.

The NHS Digital Weight Management Programme supports adults living with obesity who also have a diagnosis of diabetes or hypertension or both, to manage their weight and improve their health. The NHS Digital Weight Management Programme is a 12-week online behavioural and lifestyle programme. People can access it via a smartphone or computer with internet access.

This resource has been developed in partnership by North Yorkshire County Council, Harrogate and District NHS Foundation Trust and York Teaching Hospitals Trust, to support professionals in navigating the children and families obesity offer in the County. The resource builds on good practice and offers evidence-based standardised guidance for practitioners to help them identify and raise the issue of overweight and obesity, deliver healthy lifestyle brief advice, and know how and where to signpost their service users for further information and programmes that help them achieve and maintain a healthy weight.
